My Health Nurse is a private provider of quality nursing services to general practice and the community.
My Health Nurse Provides a mobile nursing service to the general community providing services such as wound care, catheter care, medication management, health assessments, chronic disease management, assistance with lifestyle modification, weight loss, palliative care, hygiene assessments and much more. We can also assist with case management for complex care clients requiring co-ordination and referral to other services.
 We aim to provide a safe flexible option for nursing care in the community, providing immediate nursing care with no waiting periods. There is no minimum booking period and we are happy to attend care until public funded services are available.
We offer a professional support service to general practice specialising in health assessments and care planning including, TCA's and GPMP's. We also offer the ability to organise and conduct specialist nurse clinics to assist general practice to provide a holistic health care service to their clients.
Our clients include private individuals, care package provider organisations, hospitals, and Medical Centres. |
